Abstract
Methods: We compared the number of visits to the hospital casualty department one year before and after chronic patients were included in the programme (2-10 December 2010).The Wilcoxon test was used for evaluating the differences between the number of visits both years (CI 95%). We also studied the activity based costing difference for the health care delivered in the casualty department one year before and after this programme. Results: there were 90 visits to the casualty department before the inclusion of patients in the programme [case management nurse (CMN) and hospital liaison nurse (HLN)] with a range of 0-12 visits per patient. One year after the programme was implemented there were 36 visits to the casualty department with a range of 0-4 visits per patient, p=0.01. The activity based costing in Elche General Universitari Hospital (n=54) was 20.619 €; which in comparison with the costs incurred the following year 8.247.96 €, make a difference of 12.371,04 €. Conclusions: The programme for patients with multiple pathologies treated by the CMN and the HLN has had positive effects as there has been a reduction in the number of visits to the casualty department, which in turns, resulted in a better use of this department for the decompensation or their diseases
