Abstract

The cost of chronic diseases for governments is incredibly high. The changing in the patient profile and the new technologies are making possible a better management of chronic conditions. In 2011 the Elche-Hospital General Department implemented in the Outpatients Information System (OIS) the Nurse Hospital Discharge (NHD). At the same time, in the basic area number 6 a new protocol was developed in order to attract chronic patients and increase their visibility. Objective: the purpose of the study was to compare a two months period with and without the implementation of the protocol developed and observe if there was an increase in the visibility of chronic patients. Methods: A descriptive retrospective study was carried out with the data gathered from the review of the NHD from June 2011 and February 2012 (with the implementation of the new protocol) in the basic area number 6 of ElcheHospital General Department Results: the findings showed 58 hospital discharges in June and 72 in February increasing by a 5% the rate of follow-up and 10% in new ones. Appointments: the rate of patients referred to the case management nurse (CMN) was 32% in June and 45% in February. Conclusions: In the Pla health centre we perceive a nurse proactive attitude, which together with the electronic medical record, the use of telephone and the implementation of this new protocol make possible to decrease the rate of non-visible patients.
